Job Description:   **Pharmacist-Special Clinical-Infectious Disease, Pharmacy, Days, Full Time** **Description** The incumbent uses an array of professional (pharmacy) and personal (analytical, communication, supervisory, computer) skills to provide leadership in infectious disease medication-related patient care to achieve: drug use that is safe and effective; pharmaceutical care focused on achieving positive patient outcomes through cost effective drug therapy; pharmaceutical services that advance the cure, amelioration, or prevention of disease; promotion of pharmacy as an essential component of the healthcare team. The incumbent, in conjunction with the Infectious Disease Medical Director, is responsible for the development, implementation, documentation, and advancement of the antimicrobial stewardship program for JHSMH. Because this function draws on and affects clinical pharmacists to such a degree, it is critical for there to be good open communication and coordination between the Clinical Specialist and the Clinical Coordinators. **Qualifications** **Education:** Doctor of Pharmacy (Pharm D) graduate of an accredited college of pharmacy. Completion of ASHP accredited Infectious Disease Residency Program or equivalent. **RequiredExperience:** Minimum two years of clinical pharmacy practice. Clinical experience in Infectious disease and/or participation in an antimicrobial stewardship program preferred. * For those hired after November 14, 2016, KentuckyOne Health will verify education required by the position, through primary source verification, prior to beginning employment. Education verification will also include the collection of a high school diploma or equivalent, transcript, or other written validation of education. **Licensure:** Pharmacist’s license to practice in the state of Kentucky. **Job Knowledge/Skills:** Knowledge of the following: Principles, standards and practices of medical, pharmaceutical and dietary functions in infectious disease. Principles, standards, goals, objectives, and operations of antimicrobial stewardship programs. Pharmaceutical care programs Regulatory requirements applicable to the pharmaceutical care function. Practices and techniques relating to staff education and development. Ability to formulate and present therapeutic alternatives to physicians, pharmacists, nurses and pharmacy residents.
