Job Description:   **Patient Care Assistant, Stroke and TBI, Days, Full Time** **Description** This role is responsible for providing a variety of routine patient care activities and support services including general clerical and reception duties under the supervision of a registered nurse. Assists nurse, physicians, and families as needed **Recognizeschanges in patient status and reports appropriate information to registerednurse.** **Assistswith admission, transportation and discharge of patients** **Demonstratesknowledge of the principals of growth and development over the life span of thepatient to include neonatal, infant, pediatric, adult and geriatric.** **Assistspatient with dressing and undressing** **Assistspatient with grooming and maintaining proper and clean appearance** **Assistspatient with elimination/toileting needs** **Assiststhe patient with oral care** **Assistspatients in transferring, repositioning, alignment and ambulation** **Accommodatespatient’s needs through responding appropriately to patient’s verbal/nonverbalexpressions of needs** Safeguards patient’s property Keeps patient’s living areas pleasant and orderly Maintains patient’s self esteem, privacy and confidentiality of personal information. Provides patients with opportunities for independence and choice consistent with their plan of care. Assures patient’s rights are protected and that all patients, families, and significant others are made aware of patient’s rights. Measures body functions and documents care provided to patients Receives and shares information, observes and reports patients condition and any change of status to registered nurse Provides Safety, Sitter services for the patient as assigned Demonstrates and practices appropriate hand hygiene in delivery of patient care **Otherduties as assigned.** **Qualifications** **Education / Accreditation / Licensure (required& preferred):** High School Diploma or GED required. CNA certification preferred. Effective September 1, 2017, current BLS through the America Heart Association, or a previously approved provider, will be required before employment. **Experience(required and preferred):** One year nursing assistant experience in acute or long-term care or successful completion of 1 semester medical/surgical clinical experience in a nursing program or successful completion of nursing assistant program. * For those hired after November 14, 2016, KentuckyOne Health will verify education required by the position, through primary source verification, prior to beginning employment.
